The TT patch installs a no-cd version of the exe (afaik) so as long as you have a valid serial, you're fine.

And just saying 'it made the game unplayable' isn't really informative. Not informative enough to help you anyway.
If what G1D said is true and if you mean by unplayable that your cheats don't work, that is correct. 4.0 has an anti-cheat so cheats no longer work.
Otherwise please provide information on how/why it made the game unplayable (as in, what is not working properly when you install 4.0; errors? crashes?). Also make sure you read the FAQ in that sticky in relation to AV programs that may cause false-positives.
