It sounds like it might be DirectX playing up. Also, you can always try to Omega Radeon drivers, at least to determine if its a gpu driver problem.
I just checked my directx9.0c file versions and they dont match yours at all - can you run "dxdiag" and tell us what version of d3d8.dll and d3d9.dll you are running (DirectX Files tab)?
Regarding your cooling issue - you should also be able to check this in the BIOS/CMOS setup screen, or at startup as the PC boots. The (stock) fan should be pushing air onto the heatsink - although there is much discussion around which way is better with certain specialist fans.
Also check every other fan- depending on your case you might have intake fans (pushing air from the room into the case) and exhaust fans- pushing air out of the case. Need to check all of these.
Finally, as for other coolers- Coolermaster, ThermalTake or Zalman are a fairly safe bet. Must admit if your stock stuff isn't working it does indicate a problem which may only be slightly masked by better cooling.
