Where I live (greater Toronto area) McDonalds used to be terrible from the late 90s till the mid 2000s, but they all ended up renovating and doubling their standards. Everything now is made to order and relatively fresh. Although it's far from my favourite fast food place, you get what you pay (my meals there are like 4 bucks) for and it's always the same.
I voted Burger King, because for the past 5 or 6 years, I've regretted every time I've eaten there. My sandwich looks and tastes different every time, its more expensive than most places, and the overall experience is sub par.
Taco Bell is dirty, but tasty. I feel bad when I go there, but not because the food was bad or a gyp. KFC used to be delicious here, but it constantly went down hill and now I wouldn't eat at an actual KFC unless it were in a mall, where they are constantly restocking.
The best food places are the indie, non-franchise joints. I think my favourite fast food place from that list is Wendy's. I eat there every couples weeks and I don't think I've ever had a terrible meal. One of my favourite fast foods in general is Harvey's, which they don't have in the U.S, but it's popular in Canada and they grill all their burgers. You also get to choose all your toppings, and they give you a pretty thick patty.

I'll second BK as being a pretty bad place. The stuff is pretty expensive, and I've found the food to be mediocre on the few occasions I've been, whether it was the one near Niagara Falls, or the one in downtown Vancouver. Even the 'Hungry Jacks' in Australia, which is their version of BK, sucked.
I haven't had America's version of A&W, but the Canadian one is pretty good. They always have decent deals every week or so. Dairy Queen's burgers are also really awesome, and priced well.

I hear of Texas having some of the most unhealthiest foods and people.
Everything's bigger in Texas.
But, then again I hear..

Yeah I know the Burger King by Niagara falls, because I'll go to Niagara falls often (it's an hour away from here). It's pretty crap, but surprisingly better than other Burger Kings around here. But there's a Wendy's right across the street.
A&W is good stuff here as well, my only problem is with the price.

I've actually had better experiences at BK than most other restaurants(In-N-Out excluded from this list).
And the picture Dover linked was of In-N-Out. They are very, very good.. Everything is cooked fresh, you can pretty much watch them cooking it. Also, they're cheaper than most other restaurants...
Unfortunately, they were only located in the three most southwestern states of the US last time I checked.

Burger King > McDonald's... There is no competition there anymore tbh.
The only thing McD's has over them is about a 20:1 ratio. Someone should make a Xzibit poster about a McD's in a McD's...
McD's used to have a double cheese burger, medium fries, and a 42oz drink on their $1 menu...
Now they have a "McDouble" burger which has 2 patties and only 1 slice of cheese... what kinda fucking retardation is that, whoever came up with that idea should have been fired, but I bet they got a raise for saving those cheap fucks more money.
The small fry is now on the $1, but that is just like 10 fries... And the 42oz drink is now maybe $1.50'ish, and I think the medium drink is still over a $1. I can't remember prices anymore, I only eat there when I am on my way to work and desperate for breakfast.
Burger King's burgers are not the best, but they are easily 10 times better than that crap McD's calls meat.
The fries are about a tie between them, but if I had to pick I would say BK's are better.
----
Wendy's fries are normally like raw potatoes dipped in grease till barely warm, then they pour half a shaker of salt on them.
Their Burgers are decent though.
----
Taco Bell is awesome. I only eat their bean burritos w/o onions, chili cheese burritos, and the occasional chicken quesadilla... but I could eat there breakfast lunch and dinner for weeks and only get sick of the gas.
----
Hardee's > all (in this part of America). They are obviously a lil more expensive, but when you take a bite of that real angus beef thick burger, and then reach for a couple of curly fries... you will know why you paid the extra money.
Their burgers are so thick and filling I don't even bother getting a double.
----
Sonic has some pretty greasy burgers, but they are still good for change sometimes. Their fries are nothing special, but their tatter tots, cheddar bites, or mozzarella sticks are very nice replacements.

Over here the McDouble was introduced long after the double cheeseburger's price was bumped up. I get it instead of the double cheeseburger, it saves me 30 cents and the calories, and tastes the same.
And man, it seems like a lot of Americans here say Burger King is a lot better than McDonalds. I wonder how it's like down there, cause its not even a comparison up here, at least not anymore.
