I used to have this happen on my server, this was like well over a year ago (before the hacks & exploits)

I used to run a 10 player server on a 512/256 connection. Yeah I know, it just can't hack it! But at that time, I didn't really know, which is where I learned my mistakes.

Within 5 mins into the game, 8 people joined (me included) and it was laggy as hell (as you can imagine) then the 9th person joined, which cause my PC/connection to not cope with it, and I also got similar results to the above logs.

So, maybe it could be the connection/PC not coping??

Just a guess...
